Registration:

Please bring your physician’s order, insurance information and 2 forms of personal identification (driver’s license, SS card, etc) to the hospital.  All patients must first complete a hospital registration prior to treatment. Registration allows for gathering of your physician’s order, insurance information and proper patient identification to ensure your safety during your visit.

How do I make an appointment for Ambulatory Medical Unit (AMU) Procedures?

A patient or a physician can make an appointment for the Ambulatory Medical Unit by calling 252-449-5606, Monday through Friday, 9:00 a.m. to 5:00 p.m. The patient must be registered at the hospital, have a copy of the physician's order and provide insurance verification.

How do I get a copy of my medical records?

If you wish to obtain a copy of your medical records the day of your visit, please notify any one of your care providers and we will make arrangements for you to receive your records that day.  To obtain a copy of your medical records any time after your visit, please call our Medical Records department at 252-449-4520. 

Please keep in mind that due to regulatory requirements, hospitals must keep a record of anyone receiving a copy of your medical records (including you!). You will be asked to complete a “Release of Medical Information” form prior to receiving your records.  This is a measure taken to protect your privacy.
