Patient Information

Here are some simple things you can do to take an active part in your health care.

(For this and more information, consult our patient, family and visitor guide which covers important information — directions, visiting hours, food services and hospital policies on issues such as safety and privacy. You will receive a copy when you are admitted, or you may download the guide.)

Things to bring with you

  • Advance directives (living will, healthcare power of attorney, advance instruction for mental health treatment) 
  • Information for discharge arrangements (who will pick you up from the hospital, where you will go after you leave the hospital) 
  • Phone number of your emergency contact 
  • Insurance cards and forms 
  • Any information from your physician 
  • List of current medications 
  • Immunization information 
  • Personal items (toiletries, robe, slippers, pajamas - before bringing food from home/outside please contact the nursing staff) 
  • Reading material 
  • Two forms of Identification 
  • Applicable deductible payments 

Valuables and Personal Items

If you brought valuables such as jewelry or cash to the hospital, please send them home with a family member or friend. If you must keep valuables with you, ask your nurse to lock them in the hospital safe. You will be given a receipt for them. The hospital is not responsible for valuables and cash kept in your room. Speak to your nurse when you are ready to have your valuables removed from the hospital safe.

Advance Directives

Many patients have drawn up legal documents called Advance Directives to prepare for a time that they may not be able to make health care decisions for themselves. If you have Advance Directives, please bring them with you when you come to the hospital. If you have not taken this important step to protect your wishes concerning your medical treatment, you can find advance directive forms including living will, health care power of attorney and advance instructions for mental health from the North Carolina Secretary of State. Medical Records

You, as our valued patients, have a right to your personal health information. We have a process in place that both protects your information from getting into the wrong hands and still allows for easy access by you and those you designate as your representative.

Please follow these steps for ease in obtaining any healthcare information/medical records: All requests for healthcare information or copies of healthcare records require a written authorization completed by you or your legal agent/representative. Two forms of identification must be presented to ensure you or your legal agent is the only person(s) obtaining your records. Contact our Health Information Department by calling 252-449-4520 during the hours of 8:00 am to 4:30 pm Monday thru Friday, or by mail: Health Information Department, The Outer Banks Hospital; 4800 S. Croatan Highway; Nags Head, NC 27959.

Should you need to request copies of your medical record information for yourself or another party, you must sign a Release of Information Consent Form  and submit it to the Health Information Management Services Department. This form must be completed and can be faxed to 252-449-4521 or mailed to the following address:

Hand-written consent will be accepted only if it includes the patient's name with the date of birth and social security number, the name of the hospital releasing the records, the name of the person/entity the records are to be released to, the dates of the services to be released, the types of documents requested (i.e. films, lab work records, etc.), the signature of the patient or legal guardian, a witness's signature, and date of the patient's signature. 

Due to the confidential nature of personal health information, records cannot be faxed or sent via email as these are not secured means of communication. Requested health information can be mailed to the address as indicated by you on your consent form or picked up at the hospital. When picking up records, two forms of identification are required.
