Rehabilitation Services

Our Mission: To provide high quality, effective rehabilitative services in a patient, family and safety focused environment.

Our team of multidisciplinary therapists are specially trained and licensed to provide a wide range of diagnostic, therapeutic and rehabilitative services to patients from pediatric to geriatric stages of life. Our therapists focus on development of care plans which will ultimately help the patient gain or regain lost skills/abilities.

Available Services include:

• Physical Therapy
• Occupational Therapy
• Speech & Language Pathology
• Vitalstim certified (dysphagia/swallowing therapy)
• LSVT (Voice Therapy for Parkinsons Disease)
• Modified Barium Swallow Evaluations
• Lymphedema Therapy

Outpatient Hours:

Outpatient hours are Monday-Friday, 7:30 am – 4:30 pm. To schedule an appointment, please call 252-449-4560 during outpatient hours.


Please bring your physician’s order, insurance information and 2 forms of personal identification (driver’s license, SS card, etc) to the hospital.  All patients must first complete a hospital registration prior to treatment. Registration allows for gathering of your physician’s order, insurance information and proper patient identification to ensure your safety during your visit.

To pre-register for an upcoming Rehabilitative Services visit, please call 252-449-4560 between 8 am and 4 pm.

How do I get a copy of my medical records?

If you wish to obtain a copy of your medical records the day of your visit, please notify any one of your care providers and we will make arrangements for you to receive your records that day.  To obtain a copy of your medical records any time after your visit, please call our Medical Records department at 252-449-4538. 

Please keep in mind that due to regulatory requirements, hospitals must keep a record of anyone receiving a copy of your medical records (including you!). You will be asked to complete a “Release of Medical Information” form prior to receiving your records.  This is a measure taken to protect your privacy.
